REQUIRED: Please list below all children going on the mission trip.
OPTIONAL BUT RECOMMENDED: If your child is between 12-17 we would love to hear from them directly as to what they’re looking forward to and why they would like to be a part of the mission. If you are comfortable with giving them this option, and allowing us to get to know your family a bit better, please provide their emails below, and check off the "Fill out minor questionnaire". This should only take your child 5-10 minutes.
These are the questions that will be asked:

- 2. How would you explain to someone how to become a Christian?
- 3. What is your understanding of the Holy Spirit?
- 4. What gifts or abilities has God given you that you hope to use on this trip?
- 5. How do you feel about initiating a conversation with someone about
- 6. How are you feeling about being a part of this mission?
The environment you experience on a mission will likely be different than usual life. Some of the unique conditions are listed below and we'd like you to evaluate any reaction you may have, or accommodation you wish to seek in order to participate in a mission with FamilyLife. While we will do our best to make appropriate accommodations for your family, please know that by their nature, mission trips come with some risks and can bring some stress to the surface (especially for younger kids):
The following are some conditions you may encounter on this mission:
* Housing without elevators, or air conditioning
* Extensive walking
* Full and/or very demanding schedule (consider how this might affect young children’s nap times and sleep schedule)
* Food that is prepared for the whole mission team. You may not have control over what food is available.
Please note: A pre-existing medical condition does not necessarily disqualify you from being accepted to participate in a mission, however it is a necessary component of the application process to ask these questions. Know that we may not be able to accommodate all your needs. If you are on medication, you will need to bring enough to last the entire time you are gone.
